Q1 - How do I request a tenure-eligible faculty position?   Occurs one-time per year (early fall) and through a separate process from this form.  Follow steps sent via communication from the Office of the Provost for annual submission of college-wide requests
Q2 - What happens when a tenure-track faculty member resigns or retires?
  If the resignation or retirement is before the annual submission (see Q1), submit a request through that separate mechanism.   Resignations and retirements that occur after the annual submission date, should be submitted the following year. Interim career track faculty coverage can be requested through this form if needed (see Q3 and Q4)
Q3 - Should deans email the Provost of Vice Provost of Faculty Affairs about resignations or retirements?
  No. See Q1 and Q2
Q4- Who submits the Google form?
ADOs, after consulting with their dean and receiving approval.  All requests are considered as coming from the Dean and Deans must be fully aware of a details related to each request.
Q5 - When should I use this Google form?
  1- All faculty requests (replacement or new positions), 2- All new staff requests, 3 Title or role changes for a staff replacement, 4- Interim/temporary career track replacement (see Q2), 5 any request that requires TRIF funding (AWD or VPR)
Q6 - When is the Google form not required
?  A. Grant funded staff position (submit an ePost), B. Staff replacement (title and role does not change) (submit an ePost), C. Out of state employment (dean signs, and routes for signatures), D. Waiver of recruitment for a position that has already been approved to be replaced through this google form.
Q7 - Other important considerations: 
Colleges have internal processes for their own personnel requests, work with dean to understand approach(es).   This FAQ guide is subject to change and will be updated/communicated when needed.

Search:
This should be the most common request and is a request to complete a competitive search process that is inclusive of a diverse pool of highly qualified candidates from both inside and outside of NAU. 
NOTE: If NAU only (internal) search is requested for any position, please note that in the rationale and provide a reason for the request. 
NOTE:  Requests for out of state work assignments can be signed by the dean and sent directly to HR.  This form is NOT needed.

Replacement or "NEW" positions
Any position request that is not currently listed in your FY 23 budget (state, local, or other account) is a new position request. 
REMINDER
: For Faculty replacement requests, please list the name and position number (PN) for the person being replaced. If it is a new position, please verify that your College Dean has already discussed this request with the Provost or Vice Provost for Faculty Affairs or Vice Provost for Academic Operations prior to google form submission and have approval and budget identified to request a new position.  
Reminder:  Direct "one to one" replacement requests for staff position where the title and role will remain the same do not require this form (see Q6 above).  Submit the ePar term and then the EPost for replacement.  The ePost will be reviewed for budget and other considerations.

Position Title: Faculty titles: please provide the series (T/TT, Clinical, of Practice, Teaching, Lecturer) and rank requested.  The rational should clarify WHY this faculty track is appropriate and what rank is being requested (i.e. assistant only vs assistant or associate vs open rank).  This choice should be based on the needs of the position.  If there are budget considerations with this choice, please work with the Provost, or VP-FA prior to submission.  Staff/Service Professionals: please provide NEWLY designated NAU HR job titles.

For Staff/Service Professionals,
please look at the following site: Class and Comp and use the most recent NAU HR CLS job profiles to guide salaries.  You only need to list the HR approved the "pay level" (I.e. Pay level 103).  For Faculty, list the range of all current faculty in your unit at the same title/rank AND consider the current CUPA goal for that title/rank.  For planning purposes use only the bottom of the green and the top of the yellow zone from your college CUPA data to generate the range for salary.  These data were provided to all Dean's offices.

For the funding source, please clarify if funds exists now and from what source.

Special notes on request (please make this section brief)- Typically, this will be the name and position number of the person being replaced (i.e. "Replacing: John Smith - PN: 02134") or the name of the person identified for the waiver hire (i.e. "Waiver for Lou Lumberjack).

This is the key part of the request.  Please DO NOT attempt to format this text (no bullets or other formatting).  It makes the resulting spreadsheet problematic and can result in your rationale not being readable.   This form generates a spreadsheet where this text will be in one cell.  Un-formatted text is best, but please do include spaces between words...
Please provide the key reasons this position is needed.  Please write in 3rd person and focus on the unit's needs that are being met by the request.  For faculty, provide the workload plan for the position (base expectation is 4/4 for CT or 3/3 for TT).  If less than base expectation is planned please explain why.  Listing of specific courses is NOT needed and does not help the rationale. 
At the Dean's office review, it is expected the Dean is confirming both the ongoing need for the position AND the priority of this need relative to other needs in the unit.  This should also be clear in the rationale.
