Statement of Attestation

As the parent/caregiver of the above-named student, I hereby confirm and attest that my student:

 does not possess and/or bring a cell phone to school 

                                           or

agrees to have their phone locked in storage in the main office for the entire school day (8:30-3:05)

I understand and agree to adhere to the school's policy regarding the use of personal electronic devices on school grounds.

I also understand that any communication with my student during school hours must be conducted through the school's main office, following established procedures for student contact.

I further understand and acknowledge that if my student is found to have a cell phone on school grounds, it will be confiscated and consequences will be enforced according to the school's policy.
