Due to COVID-19, we have implemented a variety of safety and precautionary measures to help ensure the safety of our customers and team members. This includes a two-step check-in process:

1. This agreement is the first part, covering our new protocols and your agreement with them. We must refuse entry to anyone who has not submitted this form in advance of their entry to TCS.

2. You will be REQUIRED to full out a Same-Day Health Screening prior to entry when you arrive at TCS. (QR code will be at the TCS Main Entrance Door).

In addition to this acknowledgement, clients entering the facility will fill in a health self-screening once per day before entry and be subjected to a temperature screening. Anyone exhibiting a 100.4 degree fever or above will not be allowed in the facility.

Crew and production must enter TCS through the main door only. It is not permitted for non-TCS staff to entre the facility through the loading dock for any reason.

COVID-19 testing is not permitted inside the TCS facility especially and including in the public lobby of the building. If you are booking a test for your crew working at TCS you must arrange for an alternate off-premise location.

Clients entering the facility or picking up must follow NY State regulations: Wear a mask covering mouth and nose and practice social distancing in compliance with NY State regulations. Unacceptable masks: masks with air vents; scarves, bandanas and neck gators.

Pickup person will be required to present ID at TCS when picking up.

All pickups will be no-contact and done at the TCS loading dock.

The returns and quality control process (missing and damaged items check) may take longer than normal.

Gear swaps during prep and after pickup may take longer than usual.

No on-set visits or on-set troubleshooting

For the safety of our employees and community, if you submit this form but fail to comply on arrival we will need to refuse your entry or pickup until the above-stated conditions can be met.
