2020 Summer Camp Withdrawal Request Form
If you wish to withdraw from the 2020 Summer Camp, please fill out this request form. Once the request is processed as per the policy below the Summer camp team will notify you via email. The refund process may take up to 3 weeks. Please add the email below to your contacts so you receive all the email correspondence sent to you.
Withdrawal On or Before June 1st, 2020: Full Refund (less $100 administration fee per child)
Withdrawal On or After June 1st, 2020: No Refund
The administrative fee can be waived if you are experiencing financial hardship. Please let us know below.
Please note that violation of any of the terms and conditions of the camp will result in the cancellation of enrollment without a refund.

List the names of the child(ren) you would like to withdraw from the camp. Please insert a comma between the name of each child.

Please provide the current and updated mailing (postal/street) address as all refunds will be directed to this physical address.
