Welcome to the John Sevier Middle School Band Program! Fill out this form to officially accept your spot in the 2025-2026 Sevier Band!

The first section is student info, followed by parent info. By completing this form, you are accepting your invitation into the Sevier Middle School Band (CONGRATULATIONS!) and will be placed in the correct class for your instrument.

All parents/guardians agree that any information provided is allowed to be shared with band staff and booster parent volunteers for the purposes of communication and general operation of the Sevier Band and Booster Club. Parents also agree to allow pictures/video of their student to be shared via social media or any other marketing material from the band.
