Omaha Street Soccer Fall League Player Registration
We are excited to begin our 1st North Omaha Soccer League this Fall! For this first year, we will offer a 2nd/3rd grade league and a 4th/5th grade league. As our volunteer base grows, we are hoping to expand and cover 1st-8th. Volunteers coaches and team managers are critical to the success of the program, so please consider volunteering. We will have support to help you along the way.

All activities will be held at Benson Park near the Dill Field Softball Complex
Practices will begin the week of August 23rd
Games will begin the week of 9/6 and end by 10/30
Tournament 10/2-10/3

Liability Waiver: Athletics presents certain inherent risks and hazards, which the Participant and parent/guardian are urged to consider and which the Participant assumes. Participation includes possible exposure to and illness from infectious diseases including but not limited to MRSA, influenza, and COVID-19.To the best of my knowledge, there are no physical or other health-related conditions, which will interfere with my child’s participation unless noted above. I, the undersigned parent/guardian for the above named Participant, understand and acknowledge that such activities have inherent risks, dangers and hazards, foreseeable and unforeseeable, that may result in injury, illness or property damage, and on behalf of myself, my family, agents and contractors, I hereby release and agree to hold harmless Omaha Street Soccer, its sponsors and its volunteer coaches, managers, club officers, directors and venues from all claims, actions, or losses related thereto. Omaha Street Soccer assumes no liability for injury or damage arising from the results of participation of the above Participant. I covenant, warrant and agree that I will examine the Facility and that if I observe or witness any significant danger, hazard or other unsafe condition, I will remove myself from participation and bring such danger, hazard or other unsafe condition to the attention of the nearest OSS referee, official or other person conducting the Activities.  In consideration of participation in Omaha Street Soccer  the undersigned agrees that their likeness, or the likeness of their child/ward may be photographed or videotaped and that such image may be published in an outlet used to promote or publicize the sports program. I HAVE READ THIS AGREEMENT, FULLY UNDERSTAND ITS TERMS, AND ACKNOWLEDGE AND AGREE THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT. I HAVE BEEN AFFORDED FULL OPPORTUNITY TO NEGOTIATE THE TERMS HEREOF. I HAVE SIGNED THIS AGREEMENT FREELY AND WITHOUT ANY INDUCEMENT OR ASSURANCE OF ANY NATURE.
