Please rank the following spaces from 1 to 3- 1 being the most preferred.  Every effort will be made to give you the desired space; however, we work hard not to place similar crafts too near each other.  Spaces are first come first serve. Cedar Springs Middle School has two stories - some booths may be upstairs *

I understand and agree to the above conditions. Payments will be completed online or via check. Once your invoice is sent you will have 5 business days to submit payment before your spot will be opened back up to the next person.   No refunds. Please type your signature to finalize the form. *
