I give permission and consent to allow photographs, videotapes, and interviews to be taken during the Day Camp session. I further give consent that any such images or interview may be published and used to illustrate and promote the camp, the church and the National Lutheran Outdoors Ministry Association. I understand and am responsible for transportation to and from Day Camp. *

Field Trip Permission (1st-6th grade): We plan to take a trip to Camp Omega in Waterville on Wednesday, July 30th. Campers will travel, in transportation arranged by the congregation (bus). At the camp, the children may participate in devotions, games, swimming, and boating. Certified lifeguards supervise all water activities. I hereby give permission for my child to participate in this day trip sponsored and supervised by Camp Omega and volunteers from the congregation.
