By my acceptance below, I (we) hereby request to either observe, participate in training groups, or participate in classes offered by Capital Dog Training Club of Washington, D.C., Inc., (CDTC) and in consideration of the acceptance of this request to observe or participate in training groups and/ or classes, I (we) agree to hold the CDTC, its members, directors, officers, agents and volunteers harmless from any claim for loss or injury which may be alleged to have been caused directly or indirectly to any person or thing by the act of any person, owner, guardian and/or their dog while in or upon the training site, grounds, the surrounding area thereto, or trial site rented and/or operated by CDTC.

Additionally, I (we) acknowledge that COVID-19 and other contagious illnesses can spread from person-to-person contact. Participating in dog training activities can spread these illnesses. Understanding this, and by my (our) acceptance below, I (we) acknowledge the risk that I (and my family) may be exposed or infected by COVID-19 or other contagious illnesses by attending events, classes, or services at CDTC. I (we) also understand that exposure or infection may result in personal injury, illness, permanent disability, and death. I (we) voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to myself or my family, including but not limited to, personal injury, disability, and/or death, damage, loss, liability, or expense, of any kind, that I or my family may experience or incur in connection with any events, classes, or services at CDTC.

I (we) also understand that my dog should be properly vaccinated, as recommended by my (our) veterinarian. I (we) further understand that the rabies vaccine is required under the law. I (we) hereby attest that my (our) dog(s) has/have received the rabies vaccine and that while there is no need to currently provide evidence of such, there could be a time when it will be required.

I (we) hereby also acknowledge that observing dog training and/ or working with dogs is inherently dangerous and risky. I (we) assume the sole responsibility for that danger and risk and agree to indemnify and save the aforementioned parties harmless from any and all loss and expense (including medical expenses, property damages, legal fees, or other associated damages or fees) by reason of the liability imposed by law upon any of the aforementioned parties for damage because of bodily injuries or property damages sustained by any person or persons, including myself or my dog, whether such a loss, damage or injury be caused or alleged to be caused by the negligence of CDTC, aforementioned parties or agents or other such cause. I (we) release, provide a covenant not to sue, discharge, and hold harmless CDTC, its instructors, agents, and representatives, of any claims, including liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating to the aforementioned risks. I (we) understand and agree that this release includes any claims based on the actions, omissions or negligence of CDTC, its instructors, agents, and representatives, whether a COVID-19 infection, other illness, or damages related to dog training and/ or working with dogs occurs before, during or after participation in any CDTC events, classes or services.
