Briefly describe the impact of COVID-19 to the organization and anticipated benefit of receiving a loan from the Paycheck Protection Program. Note that, according to the CARES Act, loan recipients will be required to make a good faith certification that "the uncertainty of the economic conditions makes necessary the loan request to support the ongoing operations of the eligible recipient." *
Because demand to participate in the Program will likely exceed our capacity to help all nonprofits requesting assistance, we might not be able to respond immediately (or at all) to every nonprofit that completes the survey. Accordingly, if your nonprofit needs to move forward with seeking an SBA loan and/or other loans or programs in order to (a) finance its operations and/or(b) meet filing or other eligibility requirements for such loans or other programs, it should not delay in filing loan or other program application(s). By continuing to seek help through this Program, including by completing and submitting this survey, you acknowledge that you understand that NYLPI and its volunteer lawyers will not be responsible for your delay in applying for loans or programs, or your inability to receive loans or participate in other programs because of any delay. *
Required
The volunteer lawyers will offer only limited representation through this Program. They will provide brief advice to help you apply for an SBA PPP loan. They will not provide advice on other legal problems you may have. By continuing to seek help through this Program, including by completing and submitting this survey below, you acknowledge that you understand the limited scope of the legal help you will receive. *
Required
In order for the Program to work, NYLPI and its volunteer lawyers will need to exchange information, potentially including confidential information, pertaining to your organization. By continuing to seek help through this Program, including by completing and submitting this survey, you acknowledge and consent to the sharing of information between NYLPI and its volunteer lawyers. *

Thank you for taking the time to fill out the Paycheck Protection Program for Nonprofits survey. Completed surveys will be reviewed on a first-come, first-serve basis. Because we anticipate a lot of enthusiasm for this Program, it will take some time to process the surveys. We appreciate your patience. Our volunteer lawyers will reach out just to those survey respondents whom we have the capacity to assist. Unfortunately, we will not be able to let you know if we do not have the capacity to assist your organization.
