We greatly appreciate your taking the time to fill out this form if you witnessed, experienced or were requested to report conduct that violates our Social Conduct Agreement or Community Leader Agreement. This is the Form named in Firefly's Conflict Resolution and Accountability Policy, and is an important step in our accountability and conflict resolution process.

This form submits incidents to the Accountability Committee, which is a very small group of board members and staff committed to community wellness. In instances where a response is needed, a member of the committee will reach out to involved parties as needed. 

Information submitted here is considered confidential. We value the safety and wellbeing of those who may have been harmed, or are reporting conduct, and prioritize protecting their identities and information.
