Bus Stop Change Request Form
Request for bus transportation to and from a location other than the student's registered address for child care and alternate address purposes must be signed by parent/guardian and submitted to the Transportation Department. Please note: child care providers are required to bring the students to/from the bus stops at all times. Please understand services are a courtesy and is not mandatory; therefore your cooperation is needed to ensure safety. The student will be assigned to the closes existing bus stop along an existing bus route; as no day to day changes are permitted and no additional stops will be added! All request must be received at minimum (15) working days prior to the requested start date of the change. Parents, school, and bus drivers will be notified if request is approved or denied; there must be an available seat on the requested route before requests can be approved.
