Field Trip Request Form

This Field Trip Form Applies to All Trip-Related Requests, including Permission Slips, Subway Passes, School Lunches, Admission Fee Payments, Yellow Buses, and Charter Buses.

Please Note the Following Deadlines for Submitting a Field Trip Request:

For City Trips WITHOUT Buses: This Form Must Be Submitted At Least TWO WEEKS Prior to the Trip Date To Ensure Timely Request of School Lunches.

For City Trips WITH Yellow Buses (if available): This Form Must Be Submitted At Least THREE WEEKS Prior to the Trip Date. Please note that yellow buses arrive no earlier than 9:30 am for pick up, and must be back at school by 1:30 pm for drop off.

For Out-of City/Overnight Trips WITH Charter Buses: This Form Must Be Submitted At Least TWO MONTHS Prior to the Trip Date and Will Be Forwarded to Joaquin for Final Approval.

PLEASE TAKE ATTENDANCE AND SUBMIT TO THE MAIN OFFICE BEFORE DEPARTING FOR YOUR TRIP.
