Field Trip Request Form
This Field Trip Form Applies to All Trip-Related Requests, including Permission Slips, Subway Passes, School Lunches, Admission Fee Payments, Yellow Buses, and Charter Buses.

Field Trip Plans Must Be Approved by the Principal, and Should be Discussed with Your Team, Prior to Submitting this Request Form.

When submitting your trip plan to Joaquin for approval, please include a description of how the trip is aligned to your curriculum and a plan for checking student attendance at different times and locations during the trip (i.e. which staff member is assigned to which students, and when will attendance be checked).

Please Note the Following Deadlines for Submitting a Field Trip Request:

* For City Trips WITHOUT Buses: This Form Must Be Submitted At Least TWO WEEKS Prior to the Trip Date To Ensure Timely Request of School Lunches.

* For City Trips WITH Yellow Buses (if buses are available): This Form Must Be Submitted At Least THREE WEEKS Prior to the Trip Date. Please note that yellow buses arrive no earlier than 9:30 am for pick up, and drop off back at the school no later than 1:30 pm when a return trip is requested. PLEASE NOTE: IF THERE ARE TOLL FEES FOR THE YELLOW BUSES, THE TEACHER/TEAM REQUESTING THE TRIP WOULD HAVE TO PAY THE TOLLS TO THE DRIVER WITH A PERSONAL CHECK OR MONEY ORDER AND REQUEST REIMBURSEMENT AFTER THE TRIP.

* For Out-of City/Overnight Trips WITH Charter Buses: This Form Must Be Submitted At Least TWO MONTHS Prior to the Trip Date and Will Be Forwarded to Joaquin for Final Approval.

PLEASE TAKE ATTENDANCE AND SUBMIT TO THE MAIN OFFICE BEFORE DEPARTING FOR YOUR TRIP. AS PER SAFETY PROTOCOL, THE OFFICE WILL FORWARD YOUR ATTENDANCE AND TRIP DETAILS TO RAY GARCIA - SAFETY COORDINATOR.
