Team Success- Back to School 20-21 Survey
Team Success is working on plans for re-opening school in the 2020-21 school year and is currently exploring various options based on health related guidance provided by the Centers of Disease Control and Prevention (CDC), the Florida Department of Health in Manatee County, and the “Safe Smart Step-by-Step Plan for Florida’s Recovery” by Gov. Ron DeSantis. We want your feedback to understand what is most important to you!

We are all hoping that the coronavirus pandemic will end soon. However, we need to prepare for the possibility that it will continue into August 2020 and beyond. Please fill out this survey assuming health and safety measures like social distancing are still in place.

Your answers to this survey are anonymous and confidential. If you have more than one child in your family, we recommend filling out this survey for each child.

Back-To-School Plans
Plan A: Normal
All students return to on-campus learning following traditional schedules.

Plan B: e-Learning Team Success
All students will continue eLearning. Students and teachers will meet using online platforms.

Plan C: Blended Learning
Blended Learning would mean that 50% of students attend school on Monday and Wednesday and 50% of students would go on Tuesday and Thursday. The groups would alternate going to school on Friday's.

Health Measures
Nothing is more important than the health and safety of your child as we plan to re-enter school buildings this fall. We will take all available health and safety precautions to make sure that students and staff are safe in entering school buildings.

With that in mind, please indicate how important the following precautions are in sending your child back to school.

The school is thoroughly cleaned at least once a day.
Students and staff are required to wear masks during the school day.
