This is a required form for parents to complete when a student is absent from school. This form must be completed and submitted within ten days of the absence.

Please complete all information below. Absence types, per district policy, are defined at the end of this form. You can view these definitions by clicking "next" at the bottom of this page. You can submit the form once you've answered all questions and viewed these definitions.

Please note, any accompanying documentation (i.e. doctor's note, college visit form, court documentation) must be physically delivered to the school's main office within ten days of the absence and will be attached to your child's absence record.
