Research Poster Presentation Submission
In order to participate in this contest you must be an AAFP family medicine resident or medical student member of the North Carolina Chapter. Non-member submissions will not be considered.

Please use this form to submit your research presentation information to the NCAFP Foundation. Before beginning your submission, please be aware the following items are needed, you may wish to complete certain sections in a Word document, then copy/paste.

2) Title of Presentation
3) Poster Abstract (300 words or less)
4) Presentation Synopsis
5) Description for Conference App (30-70 words)
6) Final PDF of the Poster must also be submitted to Tracie Hazelett by 11:59pm the 1st of October. What is submitted on / before Oct. 1st what will be judged; posters cannot be modified for judges after that date. However, if you choose to display a different poster onsite that is acceptable.

Upon submission, additional correspondence will follow from the Academy.
