2025 Seattle Rotary Service Foundation Grant Disbursement Request

Purpose of This Form
This form is required for Seattle Rotary Service Foundation (SRSF) grantees to receive funding once a grant has been awarded. Payments for grants are not available until after July 1 of the year the grant is awarded (our fiscal year starts July 1 each year).

Instructions
All grant recipients must complete this form with the required information. This form should only be submitted after you have received official confirmation that your grant has been awarded and your project work is ready to begin.

If a district matching project, name the lead partner club.  District match projects are a partnership between two Rotary clubs - one in the country and District where the project will take place (host club) and one supporting the project (international club).  Typically, the host club provides on-the-ground assistance and expertise, and the international club provides the majority of the funding.
