Purpose of This Form
This form is required for Seattle Rotary Service Foundation (SRSF) grantees to receive funding once a grant has been awarded. Payments for grants are not available until after July 1 of the year the grant is awarded (our fiscal year starts July 1 each year).
Instructions
All grant recipients must complete this form with the required information. This form should only be submitted after you have received official confirmation that your grant has been awarded and your project work is ready to begin.
