School Transportation Opt-IN Form for 2026-2027 School Year  
In an effort to optimize resource allocation, the Copley-Fairlawn City School District is asking all parents of students who WILL utilize the Transportation Department's services to complete this opt-in form. Please be aware that all students are still eligible for transportation. However, knowing which students will be using transportation services will allow us to establish the most efficient routes.

We request that you complete this form by July 22, 2026.  
Forms submitted after the deadline may require up to 5 days for processing and route assignment.  

This form must be completed each school year for all students who will be using transportation services, including new students and those who have previously used them. Please submit a separate form for each family member who will be utilizing transportation.

Students may have one bus stop all 5 days per week (at the same address) in the morning and one bus stop all 5 days per week (at the same address) in the afternoon. Multiple pick up and drop off locations are not permitted.  


If your transportation needs change, please submit the form and contact the Transportation Department; we will make the necessary adjustments.
