As a result of recent health concerns related to COVID-19, when entering district buildings, all staff are now required to take their temperature, using a thermometer, prior to arrival. Then complete the Employee Heath Screener questions below.

The district will weigh risks for COVID-19 exposure before allowing entry and make recommendations per CDC protocols.

I do not have a fever at or above 100.4 degrees Fahrenheit.

I do not have the following symptoms: respiratory infection, fever, cough, shortness of breath or difficulty breathing that are not related to having asthma, COPD, chronic sinusitis, or other respiratory or health conditions, fatigue, muscle or body aches, headache, new loss of taste or smell, sore throat, congestion or runny nose, nausea or vomiting, or diarrhea.

I or no one in my household is in self isolation, under quarantine or exhibiting any of the symptoms of COVID-19 outlined by the CDC (fever, cough, shortness of breath, or muscle aches).
