FY20 After-School/ Saturday Food Service Request Form
* Requests must be submitted 5-7 school days prior to the start of your meal or snack program.

* Schedule food service after the regular school day has ended.

* Schools must have a warming or cooking kitchen for hot after-school meals. Receiving or unmanned schools do not qualify for hot meals.

* Choice of meals AND/OR snacks are available Monday - Friday.  

* Choose only one meal service (breakfast or lunch) on Saturday.

* A minimum of 50 meals served are required for a hot meal service. If meal participation falls below 50, your school will be switched to cold meals.

* No staff is provided for snack service or for cold meal service with less than 40 meals served. A school-designated individual will be responsible for program regulations and meal counts.
