Time Preference: When we build the schedule we will try and take teacher requests for time slots into consideration. Please mark all periods you would be available to teach your class
First Hour: 9:30 - 10:30 * I can only do this time*
Second Hour: 10:30 - 11:30 *I can only do this time*
Third Hour: 12:30 - 1:30 * I can only do this time*
Fourth Hour: 1:40 - 2:40 *I can only do this time*
I would prefer any time BEFORE lunch (1st or 2nd)
I would prefer any time AFTER lunch (3rd or 4th)
I'm good with whenever is best for the schedule
Room Requests: We will take into consideration any reasonable request for space and amenities needed for your class. Please list your preferences here. (room with sink available, tables/chairs etc)
Additional information: Please use this space for any special notes or information that you think will be helpful for us to know about your class during registration
