York Region District School Board Aquatic Leadership Program Part 1 - Student Registration Form

Ministry of Education - Ontario Secondary School Credit and Lifesaving Society Certifications:

• 1 Secondary School Grade 10 Credit – Healthy Living and Aquatic Activities (PAQ2O)
• Lifesaving Society's Bronze Medallion
• Lifesaving Society's Bronze Cross
• Lifesaving Society's Standard First Aid with CPR-C

Prerequisites:

Students must:
• be 13 years of age or older
• currently attending a Secondary School
• be able to swim 400 meters

The decision to run classes is based on student enrollment and teacher availability. Students must attend all classes. Assessments and Evaluations will occur throughout the program.

All applicants must register with York Region District School Board and the Town of Aurora. Please note that registration is not complete until payment is made to Town of Aurora.
