I, hereby give my consent for my child to participate in the ASP Clubs by B4 Academy. I understand that these activities may involve physical exertion and carry a certain degree of risk. I acknowledge that B4 Academy and its staff will take all necessary precautions to ensure the safety and well-being of all participants. By signing this waiver, I release B4 Academy and its representatives from any liability for injuries or accidents that may occur during these sessions, except in cases of gross negligence or willful misconduct. I confirm that my child is in good health and has no known medical conditions that would prevent their safe participation in these activities *
I, hereby give my consent for my child  to be photographed and recorded on video during activities organized by B4 Academy. I understand that these photos and videos may be posted on social media platforms and used for advertising and marketing purposes by B4 Academy. By signing this waiver, I release B4 Academy and its representatives from any liability related to the use of my child's images and recordings. I acknowledge that I will not receive any compensation for the use of these photos and videos. I confirm that I have read and understood this consent form and agree to its terms. *
