MTHS Transcript Release Form
This form must be submitted any time an official transcript must be sent to any institution/organization. Prior to submitting this form you should: 1) Submit your applications to the college. 2)Add the college to your COLLEGES I'M APPLYING TO section of Naviance. 3)Follow the steps in the Letter of Recommendation process, if applicable. 4) Submit this form 10 SCHOOL DAYS in advance of your application deadline. 5) Make sure you send your SAT or ACT scores directly to your colleges in advance of your application deadline.
