The goal of this form is to improve the game day experience of NorCal club hosted events. Data from these forms will be used as general information. NorCal will review and follow up with clubs that have repeat issues following the season of play (ie - after the Fall or Spring season). If you are looking for immediate attention to one of the matters in this form please contact your event coordinator. For issues related to sportsmanship, violence at the field, red cards or other serious incidents please have your club submit a PAD incident report.
