EARTH DAY 2020 - SCREENING REQUEST
Please submit this form by Monday, February 17th in order to screen the documentaries starting April 4th as part of Earth Day and Museum Day 2020.

NOTE:
- You can request to screen one or all three documentaries as part of Earth Day and Museum Day 2020
- The first screening must be on Earth Day: April 4, 2020
- Museums cannot charge guests a separate fee for the screening - it must be part of the museum admission
- Smithsonian Channel will provide marketing materials to promote the screening
- DVDs will be shipped once the signed license is received by the channel
