2024-2025 MISD Transportation -     Alternate Bus Stop Request Form
  • Please fill out one form for each child that will need alternate address transportation.  
  • This form will only be valid for the 2024-2025 School Year.  You will need to reapply each year.  
  • Short Term changes due to emergencies can be handled by contacting your child's school administrator and requesting an emergency bus change.
  • Please be as accurate as possible with all information to help prevent delays in processing.
  • Your Primary address in Skyward must match the primary address on this form.  Please contact your child's campus for information on updating addresses.
  • As backup we recommend that your alternate address person (i.e. Grandparent) is listed as an emergency contact in Skyward.
  • MISD can only transport to Daycare Facilities that are located in the attendance zone of your child's elementary school.
  • Please do not use this form to try to change your primary address. Address changes are processed through Skyward and uploaded to our system within 24 hours.
