*Returning to Conditioning Activities Survey - Student and Parent/Guardian. (FOOTBALL)
We are in the process of creating plans for limited conditioning activities for our athletic programs. Please review each question and answer carefully. This is a survey to help ensure the safety and health of our students, coaches, and school personnel.  

The planned conditioning activities are voluntary only. Participating does not guarantee a spot on the team or the program. There will be tryouts planned afterwards.

We will follow the recommendations of the the LA County Public Health Department, NFHS, and CIF.

Please keep in mind that LA County has not been approved for competition. All social distancing must be diligently maintained and the COVID-19 number must drop dramatically before that start of the CIF games and competition will be formally announced.

PLEASE READ:  If your Student has any of the following underlying conditions, it is strongly recommended they do not participate, even if they are not experiencing COVID-19 symptoms, due to the increased health risks that are associated with COVID-19 exposure for these groups:
- Serious heart conditions
- Chronic lung disease
- Moderate to severe asthma
- Severe obesity (body mass index of 40 or higher)
- Immunocompromised
- Diabetes
- Chronic kidney disease undergoing dialysis
- Liver disease
- Reside with a family member with these high-risk underlying conditions
PLEASE READ:  Even with all measures taken, coming to the campus may substantially increase you and your child’s risk of contracting COVID-19. ECRCHS has no way to control exposure that may occur to your child, particularly since so many people who are COVID-19 positive are asymptomatic and may not realize they are sick.
PLEASE READ:  Other students may be exposed through community spread of the disease at youth sports, hair salons, grocery stores, retail stores, religious observances/meetings, and numerous other locations throughout the community. Other students also may be exposed because a family member, friend or neighbor of the student went to one of these locations and was exposed.
PLEASE READ:  We currently do not have an athletic trainer on site in case of injury.  If a students exhibits any possible COVID-19 symptoms, the coach and/or staff member will be isolated and the parent be notified to pick up student immediately.

The following criteria must be followed in order for our campus to reopen for conditioning activities (Please check each box after reading): *

Parents/Guardians, if you are willing to send your child to campus for conditioning activities, will you ensure that all requirements and protocols will be followed? *
Will your child have transportation to school and back home? Students must leave immediately following the end of the workouts. No car pools will be allowed. *
If the parent/guardian will not transport the student, please let us know how the student will arrive on campus
Does the student play on a club team/travel team? *
If yes, what is the name of the club team/travel team? Where do they practice?
Has the student practiced or played with a club team/travel team and/or 7 on 7s, and/or any clinics within the last 14 days? *
ECR Plans and Procedures
- There will be 1 entrance for all (Football) students to check in - Cafeteria Gate Entrance
- All students who have cars are to park in the student parking lot and walk past the front of the school, to the cafeteria gate entrance. (Even if other gates or doors are open, all checks in will be held at that gate.
- Student will wait at entrance. There will be 3 groups of 10 marks that that are 6 fee apart. Students will check in there. Coach will escort their assigned pod to the check in table
- There will be 1 exit for students (S6/Lower B Gate). Coach will escort their assigned pod out of campus to their cars or pick up.
- Students, must wear their masks covering their nose and mouth, remain 6 feet apart from one another.
- Parents/Guardians may drop off the student in the designated gate but must depart from the lot after drop off
- Parents/Guardians may remain in the  but must stay in the car
- All students, coaches, and personnel will be screened and have their temperature taken
- Students will have to remain 6 feet apart while awaiting the coach to log the student and take their temperature
- There will a table with plexiglass, hand sanitizer, extra masks, rubber gloves, information folder, paper screening forms
- Students will be grouped in "pods" of 5 to 10 students and assigned to 1 coach
- Students will remain in that pod for the entirety of the conditioning activities, until we move to the next phase
- Hand sanitizer stations will be placed through the workout areas
- No equipment will be used or shared in the conditioning only activities
- Conditioning activities will last 60 to 75 minutes until the next phase
- Only outdoor activities and/or meetings (6 feet separation)
- Head coaches will have a practice plan
