- This form should ONLY be submitted by the Sport Committee Chair with the representative nomination (or selection) from that sport committee.

- Please review the Award Criteria to make sure your candidate meets the criteria to be considered for this award.

- All fields in this form are required fields, please be sure you have the following details known for ease of submitting your nomination:

   -  A brief description of the Nominee's accomplishments specific to this award.

AWARD CRITERIA
> This award is given for outstanding service which benefited the U.S. Ski or Snowboard Teams in 2020-21. Past recipients have included outside team support personnel, donors, journalists, movers and shakers. Most recipients have been individuals, though some organizations have been recognized.

> Recipient’s service should be to the national teams.

> Achievement should be based on the 2020-21 Season.

> You are required to submit information that documents how the candidate meets the criteria for this award. There is a maximum character count for nomination information. Please be sure to only include specific points that match the award criteria.
