It is imperative to complete this form as soon as you know you will be missing a rehearsal. Rehearsals can be rescheduled, but it is difficult to do so with little to no warning. Little to no warning is unfair to students, parents, and your directors.

Please complete this form at least 48 hours before an absence. If there is an emergency in the 48 hour time span, please complete this form and briefly explain the emergency at your earliest convenience. If you do not follow this policy, you receive a strike. Once you receive three strikes, you will be conferenced with and removal from the show or a part may occur.
