If your not a gym member, Are you interested in becoming a cheer gym member? As a Cheer Gym member you receive discounts on classes, camps, clinics and Cheer Fridays. YOU receive one 4 week 1hr class per month, Free Pass to bring a friend to cheer Friday per month. We are running a NEW YEAR SPECIAL: $50 includes 2 class sessions in the current month, annual registration fee & tshirt, shorts and bow. *
* CHEER RULES*- 1. Cheerleaders will Always Pay Attention and listen to their Coaches. 2. All cheerleaders will always show good teamwork, cooperation and commitment. 3.Cheerleaders must wear squad practice gear and mask to all practices. 4. No gum chewing and no jewelry will be worn at practice. 5. Cheerleaders will arrive to practice at least 5 minutes before start time. 6. Cheerleaders will treat teammates and coaches with respect.7 All cheerleaders will be responsible to keep their practice gear. 8. All cheer class or practices are mandatory to attend. 9.Each Cheerleader is expected to follow the squad rules and guidelines. 10.Cheerleaders will not be allowed to be on phones during practices. Phones will be collected at the beginning of practice and given back at the end of practice/ games. 11.Cheerleaders will not participate in any negative gossip or communication that may adversely affect PZC. This includes but limited to: verbally to other individuals, email, facebook, twitter, Instagram, chat rooms or any other websites.12 Any physical altercations or bullying may result with suspension off the team or termination off the program participation. 13. There will be no stunting, hugging or touching during practices or events. The above rules will be enforced by the head coach, assistant coaches and Team Managers. Any violation of the cheer squad rules could result in termination of the program participation. Below signature constitutes acceptance of the above rules and regulations. Initial Below
PARENT AGREEMENT -CODE OF CONDUCT -Our son/daughter has our permission to be a cheerleader and participate in all cheerleading related activities with PZCLA for the upcoming season. I have read and discussed with our child the rules, responsibilities and commitments of being a cheerleader. We understand the responsibilities and commitment of being a Cheerleader. I also understand the commitment to my scheduled practice days. I also understand the role I must assume as a parent of a cheerleader and I will assist in every way to see that the rules and regulations are enforced. 2. I will not use any inappropriate language and will behave in a manner that respects myself, PZCLA, parents, cheerleaders and the sport. 3. I will support the coaches and the director in decisions made throughout the season. 4. I will not be involved in any matter of disrespect towards any members of the PZCLA coaches, parents or cheerleaders. 5. I will not participate in negative gossip or communication that adversely affect PZCLA. This includes but not limited to; verbally to other individuals, email, Facebook, Twitter, Instagram, chat rooms, or any other websites. 6. I will oversee that my cheerleader does not participate in negative gossip or communication that adversely affects PZCLA. This includes but not limited to: verbally to other individuals, email, Facebook, Twitter, Instagram, chats rooms, or any other websites. 7. I will not create any social media accounts under any reference to PZCLA or any other gym at anytime. 8. I understand that the PZCLA and coaches assume no responsibility for any accidents or injuries that might occur either at school, practice, or an event away from school grounds at any time. We understand that if an accident or injury may occur, I am responsible for taking my child to seek proper medical attention. 9. I understand that coaches & diractors have the right to suspend any athletes or parents participation in activities as disciplinary action all the way to removal of the program. 10. I understand the time commitment involved with this activity and if our child is dismissed or quits the cheerleading squad we will not receive any refunds. 11. I understand that PZCLA will place athletes in a class or on a team based on the cheerleaders evaluation and the program and I will accept the decisions of PZCLA coaches and its volunteers as final decisions. I also understand that it is sometimes necessary to move athletes from one class or team to another during the class to help with the program. 12. I understand the importance of punctuality and that participation in class practices are important. 13. I do hereby give my consent and support for our child to participate in PZCLA classes for the upcoming scheduled class and I will conduct myself with respect and integrity, displaying positive sportsmanship at all times. 14. I understand that if for any reason I remove my cheerleader form any scheduled classes, there are no refunds for the program at any time. 15. I understand that if I drop out of the program at anytime, all discounts, specials, payment plans and scholarships will reverse and I will payback any discounts, specials and scholarships given, additional fees and any remaining cost of the program. All discounts, specials, payment plans and scholarships are contingent on completing the full scheduled class ( last day) of the program. Payment is must be made at the time of dropout. 16. DROP OUT AGREEMENT- Parent understands that should you for any reason want to remove or withdraw your cheerleader from any Gym PZC classes program , all remaining fees on your account must be paid at the time of drop out. If for any reason your cheerleader is dismissed from any PZC cheer Gym program, all discounts, specials and scholarships will reverse and parent will payback discounts, specials and scholarships and is still responsible for paying any remaining charges on the your cheer account. There will be no refunds at anytime. Initial Below *
Medical Liability Release - I as parent or legal guardian give permission for my child or children to participate in the PZCLA program. I give permission to the hospital/ paramedic staff to administer immediate treatment to my son/daughter should he/she become ill or injured. In addition, I waive and release of PZCLA, coaches and volunteers from any and all claims and causes of actions arising from concerning any injuries, illness, losses or damages of any kind which my child or children or I may have as a result of or in connection with my child’s participation in the cheerleading program, and /or travel to and from practices. *
Stunting & Tumble Liability Release- I As the parent or legal guardian give permission for my child or children to participate in stunting or tumbling with PZCLA Program. I waive and release PZCLA, coaches and volunteers from any and all claims and causes of actions arising from concerning any injuries, illness losses or damages of any kind which my child or I may have as a result of connection with my child’s participation on the cheerleading program while stunting or tumbling. *
PHOTO RELEASE- I give my permission to PZCLA and its volunteers or coaches to take photos and video’s of my child or children. I also give consent to PZCLA and it’s coaches to use pictures and/ or video of my child on the PZCLA and its league websites, press release articles or social networks. Please initial below
PRACTICE GEAR/ WARM UPS- I understand that as a class member I will be responsible for purchasing PZCLA practice gear for classes. I will keep my practice uniform in a place where I can find it & free of any damages and clean for practice & all team performances during the class practice. Initial below
GYM CLASS GUIDELINES- 1. Check in and Check out procedures (Stay in car until its time for practice) Cheerleaders will check in with team coach or team mom at the beginning of practice. 2 Parents / Spectators may not sit in practice areas during practice periods. The gym is closed for spectators. 3. Attendance will be taken at entry of class practice. 4. Masks must be worn by Coaches, Team Moms, Cheerleaders, Spectators/Parents. 5. Wear Proper Attire –PZCLA practice gear, Soft Soled Shoes / Sneakers. 6. Take shower and wash all clothing after each class practice 7. Sanitize hands before/ after each practice 8. Athletes Tie Hair back slick and away from face. 9. Bring Water Bottle Clearly Marked with Name. 10 Parent understands that there will be no snacks during class practice. 11. There will be no hugging, holding hands, touching or gift giving during class practice. 12 . Avoid touching eyes, nose & mouth. 13. Groups will be broken down into total 6 or fewer team members includes Coach. 14. Participants cannot leave area/session without permission from the coach or end of class practice. 15. Athletes will stand 6 feet apart, wearing mask as per state or local guidelines. 16. PZCLA Cheer Gym provides 3 sanitizing stations. 17. NO MASK, NO PRACTICE. PLEASE INITIAL BELOW. *
COVID-19 DISCLOSURE-WAIVER- COVID-19 has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread from person-to-person contact. Federal, state, and local governments and health agencies recommend social distancing. PZCLA is taking steps to reduce the spread of COVID-19; however, PZCLA cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, attending activities could increase the risk of contracting COVID-19. By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending activities and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 may result from the act, omission, or negligence of myself and others, including, but not limited to, PZCLA, volunteers, and other participants and their families. I voluntarily agree to assume the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may incur by reason of PZCLA activity (“Claims”). On my behalf, and on behalf of my children, I hereby release and covenant not to sue PZCLA, its affiliated organizations, employees, volunteers, agents, and representatives, of and from the claims. ACKNOWLEDGEMENT: I understand and acknowledge cheer rules, parent code of conduct, drop out agreement, medical liability release, stunting & Tumble release, photo release, covid19, gym class rules & practice.
