Use this form to let us know that you would like to take your child out of school: for medical absences of less than half a day, please email the school office. If you require more than one child to be absent at the same time, for the same reason please use the same form. You are reminded that for children over the age of 5, absences will only be authorised in exceptional circumstances. You are required to give a minimum of 10 days notice for any absence request.
