Winter 2017 Alternate Physical Education Registration
This registration must be submitted no later than Wednesday December 20, 2017 by 11:59 pm.

Student must meet ALL of the criteria outlined on this form in order to earn the credit. The alternate physical education credit is for grades 9,10 and 11 only. Only ONE credit can be earned using the alternate physical education option.

• Student must participate in the alternate physical activity/sport during the same semester they enroll in the
Alternative Physical Education credit option. For example if the student is participating in a Fall sport they would sign
up for the Fall Alternate Physical Education, a Winter sport participation would sign up for the Winter Alternate
Physical Education and a Spring sport participation would sign up for the Spring Alternate Physical Education option.
Three separate registrations take place that mirrors the three athletic seasons.

• Student must participate in the activity/sport for a minimum 3 times per week for at least a total of 1.5 hours
each (excluding competition) during the duration of the activity/sport season/league.

Non-IHSAA activities must also meet the minimum participation requirements listed above to qualify.

Qualifying NON-IHSAA activities ONLY must e-mail the following information to gshafer@hse.k12.in.us:
• Roster of participation in activity/sport listing student as part of a team or activity
• Copy of practice/training schedule and game or competition schedule
IF THE ABOVE DOCUMENTATION IS NOT E-MAILED AT THE TIME OF REGISTRATION THE REGISTRATION WILL NOT BE ACCEPTED!! THIS APPLIES TO ONLY THE ACTIVITIES THAT ARE NOT SUPPORTED BY FISHERS HIGH SCHOOL. ALL FISHERS HIGH SCHOOL SUPPORTED ACTIVITIES ARE LISTED SEPARATELY BELOW.


Coach’s signature indicates successful completion of the season.
• Successful completion is defined by participation in at least 80% of the season
• Student must be in good standing throughout the season – eligible to play based on grade and disciplinary
requirements of the Coach’s expectations.
• Participating sport or activity must meet the state physical education standards to qualify. Coaches will be required
to confirm how standards were met.

Grades will be determined using the following rubric: 200 points total

• Coach/Director signature worth 100 points for completion of the season. Coach verification will be acquired by Mrs.
Shafer. Student does not need to submit a coach’s form.
• Four quizzes to be completed worth 25 points each. Quizzes will be available once
participation in qualifying activity is confirmed approximately three weeks after registration deadline.
• All work must be completed by Friday March 30, 2018 to be eligible for the alternate credit.
• NO late registrations or assignments will be accepted.


Participation in the activity/sport will be checked by the Alternative Physical Education coordinating teacher. False participation in the above criteria will disqualify student from completion of the Alternative Physical Education credit.

Once registration closes rosters for each sport will be created and then sent to each coach to confirm that the athlete is on the team. Once that is confirmed each name will be compared to previous Alternate Physical Education rosters to make sure the student has not already earned an alternate credit. The names will also be compared to the online summer school lists to confirm each applicant has not earned an online physical education credit. After each name has been checked a roster will be sent to the schools technology administration to set up participants names in Canvas and Skyward. Once that is completed the students will see an Alternate Physical Education class in Canvas and Skyward. A skylert will also be sent to confirm the student is in the class and the quizzes are available to be completed. This process takes approximately three weeks after registration closes.
