I willingly participate in the group fitness classes offered by Talitha Dance, during which I will receive group fitness instruction. I understand the nature and purpose of group fitness classes and are aware that any strenuous physical activity involves physical risks. I understand that it is my responsibility to consult with my physician prior to and regarding my participation in any fitness program. I assume full responsibility for any risks, injuries, or damages I might incur as a result of participating in Talitha Dance group fitness classes. I release, discharge, and absolve Talitha Dance, its employees, and affiliates from any and all liability or responsibility for any such accident, injury, or damage. This release shall be binding upon my heirs, executors, administrators, and assigns.
