Southwest Primary Student Intent Form: 2020-21
Dear Parent/Guardian,

Hickory Public Schools is excited to offer two options for your child for the upcoming school year. As HPS continues to focus on the academic success and the health and safety of our students and families, the school year will begin with all remote learning for students on August 17. When schools convert to blended learning, families will have the opportunity to continue in remote learning or begin instruction in a blended learning model. In a previous survey, 40% of our families were interested in the remote learning opportunity. In order to properly prepare for the remote learning program, we would like to gather additional information.

Please complete the form below stating your intent for the upcoming school year. The first option is remote learning only. The second option is the blended learning model where students will receive both in-person instruction and remote learning. Students in both programs will receive a high quality learning experience from highly qualified teachers. The blended learning option is not available at this time. We are requesting information as we are planing for the school year.

Please complete one Student Intent Form for each of your children. The form needs to be completed by July 31, 2020. Once completed, we will have your decision for the first semester of the 2020 - 2021 school year.
