There's been a discussion for sometime about food shopping for our classes. It seems like every district operates differently. This form is to collect details on the different ways we receive food orders.

Do you physically shop for those items? If so, is it in place of a duty or during your prep? 

Do you get delivery? If delivered, do you need to pick up the groceries from the main entrance or would the custodial staff help bring them to your room?

If you leave the building to shop, who is liable for your car or you personally if you get hurt? Are you liable for damages to your car or yourself? Or does the school insurance policy cover it?  *
