As your Wellness Coach, it’s important for me to understand how you view yourself and the world around you. Each person is unique, so completing this (no later than 48 hours prior to your first coaching session) helps me start to understand you and what you hope to get from your coaching sessions.  You may find answering these questions helps you clarify perceptions about yourself, what's going on in your health and life, and what you want moving forward. Think of these as “pondering” type questions, designed to stimulate your thinking in a way that will make our work together more productive. Take your time answering them. If you don't know the answer to a required question, simply state "I don't know" or "I'm not sure, let's discuss."  Your answers will be treated with complete professional confidentiality.
