 3.) DIPLOMA (Original Copy)
 4.) AFPSAT Score (Stub or AFPSAT Certificate)

For Enlisted Personnel Applicants:

 1.) Updated SOI
 2.) Military ID
 3.) AFPSAT Score (Stub or AFPSAT Certificate)

*This registration is only applicable for those who passed the AFPSAT with remarks of PASSED FOR COMMISSIONED and a BACCALAUREATE DEGREE or FOUR (4) YEARS COLLEGE GRADUATE.
