Lieutenant Governor's Volunteer Recognition Certificate
Thank you for taking the time to recognize one of Utah's many outstanding volunteers! Please complete this form as thoroughly as possible. Review the criteria below before beginning your nomination.

CRITERIA:
Narrative:
In order to receive a certificate, the nomination must include a short narrative statement that describes the nominee's service activities and must demonstrate how their service has made an impact. Narratives should be no longer than 250 words. Volunteers with identical, generic narratives will not be issued certificates.

Achievement:
The nomination must reflect the accomplishments achieved through voluntary service.

Community Need:
The service must address a need in a Utah community.

Ongoing Involvement:
The voluntary community service must be ongoing rather than a one-time activity, with the exception of outstanding service for short-term crisis situations.

Other requirements:
•Nominations must be made by someone from within the volunteer organization.
•There is no age requirement.
•Service must be performed in Utah.
•Service must be performed by an unpaid volunteer, not a national service participant.
