Please fill out this form in full for the piece you will submit to the PAC Travel Exhibition opening September 13. One work per artist, open to current PACMembers who participated in PAC travel experiences to the Netherlands, Rome, and Japan.

Please clearly label the back of your work with your name and the title before delivering! Artworks are due by Thursday, September 10.
