Enrollment Agreement

I understand that Time4U is open according to the official school calendar of the USD 379, and is closed during vacations and inclement weather days, and will not operate on early dismissal days. *

I understand the Program Staff will assume full responsibility for my child from the time he/she arrives at Time4U until my child leaves Time4U according to the written instructions for departure. *

I understand if a medical emergency arises, the Program Staff will attempt to contact me. If I cannot be reached, the staff will contact my child’s doctor. If the emergency is such that immediate hospital attention is necessary, an ambulance or emergency vehicle may take my child to the hospital. *

I understand Time4U is continually covered in prayer by our community churches. In addition, your child’s name, age and picture may be given to a Christian Adult who will pray for them individually throughout the year. *
