Team Registration Form - completion of this form is an indication that the teams listed below are intending to participate in this event.  A team entry fee (GBP 800 per team) must be paid to confirm the team entry. Please note that teams are required to be at the venue at least 24 hours prior to the start of competition and should plan to stay through the final day of competition.  Please note: no underage players are permitted at this event.
