This is the software you use to access the resource.
Which version of the resource are you using?
The format is determined by the type of software that the resource uses to function. This might be Flash or HTML5 for example. This is normally stated next to the resource link in you Learning Central module folder.

Use this section to enter details about the module of which you are experiencing the issue(s).
Resource/Tutorial Title *
Enter the title of the resource / tutorial. For example; Venous Cannulation. Alternatively, copy the link URL and paste it below.
Slide Number (or title) *
Enter the slide number (usually found at the bottom of the slide) or title slide, for example, "Learning Outcomes" or if no number (or title) is available, try to describe any identifying features, for example, "The slide with the eye diagrams".
Description of Issue *
Describe in as much detail as you can, the problem you are experiencing. For example, "tutorial freezes when 'next' button is clicked". If the problem is related to content inaccuracy, please state this below.
