This form must be completed by every parish in the Diocese regardless of whether any lay representatives or clergy attend Synod or not. This information is vital to providing an historical record of our Diocesan activities as well as helping the Diocesan administration provide accurate information in the directory and website.
There are three (3) sections required. You will need to know the basic information regarding the parish (address, website, etc.) and the Tax/Employer Identification Number (EIN issued by IRS) of the parish. You will also need to know the names of the lay members elected by the parish to represent the parish at the Synod. Finally, you will need the following general membership statistical information (totals not names):
