Elements of Risk
Although all safety precautions will be taken by Northumberland Maker staff and presenters to ensure the safety of all participants. It is important for everyone involved to understand that all programs do present various elements of risk. Incidents related to such activities may occur and cause injury through no fault of the supervisor(s) or the facility at which the activity or event is being held. The Northumberland Makers do not provide any accidental death, disability, dismemberment, medical or dental expense insurance on behalf of participants in any activity.

Should medical care be required, staff will contact the emergency number, and follow instructions. Should the emergency contact not be available, the supervisor may contact emergency services if required. I understand that any cost will be the responsibility of the parent/guardian/emergency contact.

Participating youth are expected to follow School Code of Conduct type expectation. Those that fail to do so, may result in a phone call to the emergency contact, where the youth may be asked to leave (reimbursement will depend on the situation).

I have read and understood the information outlined in this application. I acknowledge and accept the risk and assume responsibility for my son/daughter/charge for personal health, medical, dental, and accident-insurance coverage. *
I give permission to the supervisor(s) to obtain professional assistance in the event that my son/daughter/charge requires medical attention. *
I understand that I will need to sign a waiver on the first day of camp. *
If Additional supervision is required before and after camp, there will be a charge of $10 an hour. This must be pre-arranged and must be after 8:30 am to 3:30 pm. *
If I need to have someone other than the main contact/emergency contact - I understand that I need to let the instructors know as soon as possible. *
Thank you for applying to camp.
We are going to put camp requests using the timestamps to help us organize things. If you do not pay for the camp within five days, we may not hold your space. We will create a waitlist if needed. Last minute cancellations will only be reimbursed if we are able to fill the space with a waiting applicant.
