Back to school supply request form
At this point, we can’t promise that new requests for school supplies will be filled for the 2025 school year. But you’re still welcome to submit a request—we’ll do everything we can to make it happen. If we’re able to fill it, we’ll let you know right away.

Each STUDENT WILL NEED A SEPARATE FORM- If you are registering supplies for 3 different students, you need to fill this form out 3 times.

Jerrol's annual Back to School Supply Drive is intended to help local students and families in Kittitas County who, for any reason, may not be able to provide their own school supplies. This form may be filled out by anyone who knows of such a child in need, or a student themselves. These forms will be anonymously given out to our customers to "adopt" the student and fill their school supply needs. You will be contacted when the bag of supplies is ready to be picked up, beginning in August. Please fill out the form for each individual child. So if you are wanting to get supplies for 4 different kids, you will fill out the form 4 times. Requests will be filled on a first come, first serve basis, and we will try our hardest to fill every bag, but we will let you know if the demands are greater than what we can provide.  We will contact you via phone and/or email.

This year we have partnered with CASA, Kittitas Valley Health, and APOYO to be able to help more students than ever!

All personal information collected in the first part of this form is strictly to contact you when it is time to pick up supplies. Jerrol's will not share this information with anyone.
