Membership Hold
Please fill out this questionnaire to place your membership on hold. You must submit your hold request 7 days prior to your final day of attending classes. Members are welcome to place their membership on hold at anytime by filling out this form. Membership holds are available for any length of time between 7 days and 60 days.
