This form allows you to request changes to your account. Use it to add or remove family members, freeze and unfreeze your membership, or cancel your membership.

The New Orleans JCC drafts monthly dues on the 15th of each month. In accordance with your membership agreement, cancellations require 30 days written notice. Therefore, if you want to cancel as of December 15, written cancellation (via this form) must be received by November 15. All other account changes must be made by the 10th of the current month in order to process your request before the automatic draft on the 15th.
