This enrollment form is only for Cache Students currently enrolled in 5th and 6th grade.

Afterschool CAMP hours: 3:15-5:30 p.m. COST: $40/month

Payments due before the FIRST day of the next month.  
For example, the first payment would be due no later than August 31st.  If payment is made on September 1st or later, a $20 late fee will be charged ON TOP OF the required monthly fee.

PLEASE BE ADVISED:  If 2 late fees have to be applied for not meeting the payment deadline, your child will be dismissed from the program for the remainder of the 2019-2020 school year if another payment deadline is missed.  

*The monthly fee is based on monthly program participation and NOT based on daily attendance.
  Scholarships are available. After completing the following enrollment form, you will be notified after we have reviewed all applications and told if your student has qualified for a scholarship, which would allow him/her to attend for free.
   
Those who do not qualify for a scholarship will be notified as well and then given the opportunity to withdrawal from the program (at no charge) or continue with first payment due by August 31, 2019.  

    ***For those not qualifying for a scholarship, any days the student has attended prior to notification, will not be charged to the student if the decision is made by the parent for their student to no longer continue to be enrolled in the program.****
