EMS Choir Student Registration Form 2018-19
This form is intended to collect demographic information from all EMS Choir students to make logistical planning and correspondence more efficient. In addition, this form contains parent/guardian and student agreements and releases pertaining to various choir operations. Due to its contents, both student and parent/guardian should be present when filling out this form.

Completing this registration form is mandatory for all enrolled members of the Eisenhower Middle School Choir Program. Please complete this form after reviewing the current year's EMS Choir Student and Parent/Guardian Handbook. Thank you in advance.
